This position will work as part of the Crisis Rural Jail Diversion Services (CRJD) as well as be crossed trained to help with short term case management and follow ups. This position will provide crisis follow up and relapse prevention services to the CRJD crisis customers to help them avoid re-occurrence of crisis. Crisis relapse prevention services are designed to be intensive support services that include coordination of crisis services, ongoing risk assessment, education on effective treatments for mental health and substance abuse disorders, problem solving and skills building to reduce precipitating crisis factors and engagement/linkage with mental health and substance abuse services. This person will review all CRJD crisis logs and complete follow ups either by phone or face to face to insure connection with appropriate resources, as well as to make sure the crisis event has resolved. This position may carry a small caseload of short term customers that need access to services and then will help them transition to community providers. This will include providing short term case management with non enrolled customers in our Respite Facility. This position will collaborate with CRJD to assist with crisis assessments and shift coverage as needed in support of the Crisis Rural Jail Diversion Team. This position will interface with community stakeholders and maintain a professional relationship and boundaries with stakeholders in all interactions.
Requires a Bachelors degree from an accredited college or university in a social, behavioral, human service or health-related field including, but not limited to, psychology, social work, medicine, nursing, rehabilitation, counseling, sociology, human development, gerontology, educational psychology, education, and criminal justice.
Must be eligible to be certified as a Qualified Mental Health Professional. Prefer previous mental health work experience with adult and child mental health consumers and/or some crisis service provision experience. Must possess a valid Texas Drivers License and be an insurable driver.
